Telehealth Consent

This Telehealth Consent (“Consent”) applies to services obtained from the professional entities identified in the thare health LLC Terms and Conditions (the “Professional Entities”).

Telehealth allows healthcare providers to assess and treat patients remotely using technology. This may involve transmitting medical records, photos, and other personal and health information or data through various methods, including dynamic intake forms, secure messaging, and live 2-way video and audio. The electronic systems used in the Services incorporate network and software security protocols designed to protect the privacy and security of your personal information.

Possible benefits of telehealth. Telehealth allows you to interact with your provider at times and locations that are convenient for you, without the need for an in-office appointment, including for hair-loss consultations and follow-ups.

Possible risks of telehealth. Telehealth services are not a substitute for ongoing, in-person care from your local physician. Because remote consultations may lack complete medical records or a physical examination, certain risks are associated with telehealth, including potential for incomplete or inaccurate assessment.

To support accurate diagnosis and appropriate treatment, you must provide a comprehensive and accurate medical history, including all diagnoses, treatments, medications, supplements, allergies, and prior procedures. Failure to disclose complete information may result in misdiagnosis, inappropriate treatment, or adverse outcomes.

Your provider may recommend alternative care or an in-person consultation with your local physician and may decline to provide a prescription or other treatment based on the limitations of telehealth.

Technical or internet disruptions (e.g., poor video/audio quality or service interruptions) may cause delays in evaluation, consultation, diagnosis, or treatment. If you experience technical difficulties during a telehealth session, contact your provider immediately through the platform.

Telehealth is not suitable for all medical situations, particularly emergencies or allergic/adverse reactions. In such cases, contact your local physician or proceed to an emergency room. For medical emergencies, dial 911.

While security protocols are in place, there remains a potential risk of privacy breaches and unauthorized disclosure of your health information.

Please be aware that state laws may restrict or prohibit certain telehealth services, including the prescribing of medications, in your area.

Your consent to telehealth

By using the Services, you acknowledge and agree to the following regarding telehealth services provided by the Professional Entities.

Provider Qualifications and Service Limitations

  • Healthcare services will be delivered via telehealth.
  • Your provider may be a nurse practitioner or physician assistant, and not a physician.
  • You consent to evaluation and treatment by these non-physician providers.
  • Telehealth technology and clinical protocols may evolve and may differ from what is described here.
  • No specific outcomes (including from laboratory testing, if applicable) are guaranteed. Your condition may not improve and may worsen.
  • Telehealth is not suitable for all conditions, and your provider may determine that in-person care is required.

Your Understanding and Responsibilities

  • You have reviewed the benefits, risks, and alternatives to telehealth and understand them.
  • Online treatment sessions will not be recorded by the Professional Entities.
  • Your provider(s) will assess your condition and determine, in their sole discretion, whether telehealth is appropriate.
  • By using the Services, you agree with your provider’s assessment and consent to telehealth treatment.
  • You agree to provide accurate and complete medical information, including information from other healthcare providers, and an emergency contact.
  • You understand that failing to provide accurate, complete information could negatively affect your treatment.
  • Your provider may determine that your condition requires in-person evaluation or treatment.

Consent and Withdrawal

  • You may withdraw this Consent at any time by providing written notice to your provider through the platform.
  • Withdrawal will not affect actions taken before we receive your notice, nor will it affect other provisions of this Consent.
  • Your continued use of the Services constitutes ongoing agreement to receive treatment via telehealth.

Electronic Communication and Privacy

  • The Services involve electronic communication of your personal medical information.
  • Your information will be handled in accordance with applicable privacy policies and laws.
  • Messages may be sent to the email or phone number you designate; safeguard access to those channels.

Financial Considerations

  • You may obtain medical examinations or treatment from other healthcare providers of your choice.
  • If a prescription is clinically appropriate, it may be sent to a pharmacy of your choice or to one of the Professional Entities’ pharmacy partners for fulfillment and shipment to you (which may include partners such as ChemistryRX, 950 Henderson Blvd., Folcroft, PA 19032).
  • You are responsible for all costs associated with the Services, including consultations, prescriptions, and products. The Services are offered on a cash-pay basis and are not billed to Medicare or other government or private payors, and you agree not to seek reimbursement from such programs for Services received through the platform.

BY USING THE SERVICES, YOU ACKNOWLEDGE THAT YOU HAVE READ AND UNDERSTOOD THIS TELEHEALTH CONSENT AND THAT YOU VOLUNTARILY CONSENT TO RECEIVE TELEHEALTH SERVICES FROM THE PROFESSIONAL ENTITIES VIA THE thare health LLC PLATFORM.
